A normal TSH does not rule out a thyroid problem. TSH only measures whether your pituitary gland is asking your thyroid for more hormone. It says nothing about whether that hormone is being converted into a usable form, or whether it’s actually reaching your cells.

 

You’ve had your thyroid checked. More than once, probably. The number comes back in range every time, and every time you’re told that means your thyroid is fine.

Meanwhile you’re exhausted by 2pm, your hair is thinning, you’re cold when everyone else in the room is comfortable, and the scale won’t move no matter what you change.

Something isn’t adding up, and it isn’t your imagination.

What TSH Actually Measures

TSH stands for thyroid-stimulating hormone, and despite the name, it isn’t made by your thyroid. It’s made by your pituitary gland, sitting in your brain, monitoring how much thyroid hormone is circulating and sending out a request for more when levels run low.

A normal TSH tells you the request-and-response system between your brain and your thyroid is functioning. It does not tell you what happens after your thyroid answers that request.

The Conversion Step Almost No One Checks

Your thyroid produces T4, a storage form of thyroid hormone. Your body then has to convert T4 into T3, the active form your cells actually use to run your metabolism, regulate your temperature, and produce energy.

That conversion depends on selenium, zinc, and a functioning gut, since a meaningful portion of T4-to-T3 conversion happens in the digestive tract. Chronic stress interferes with this step too, pushing conversion toward reverse T3 instead, an inactive form that occupies the same cellular receptors as active T3 without doing any of the work.

Which means two women can have identical TSH results and completely different amounts of usable thyroid hormone reaching their tissue.

What Else Gets Missed

A standard panel also tends to skip thyroid antibodies, specifically TPO (thyroid peroxidase) and thyroglobulin antibodies. These identify Hashimoto’s, an autoimmune process that can be actively damaging thyroid tissue for years before TSH moves outside of range.

Estrogen dominance is another piece that rarely gets named. Estrogen increases thyroid-binding globulin, the protein that carries thyroid hormone through your bloodstream, which can leave less free (usable) hormone available even when total levels look adequate.

This Isn't Just Anecdotal

Women wait longer than men, on average, to receive a thyroid diagnosis in the first place. If you’ve been told your labs are fine and your body has been telling you otherwise, you’re not alone in having that conversation stall at one number, and you’re not wrong to keep asking.

What a Full Thyroid Workup Actually Includes

A complete picture goes beyond TSH alone:

  • Free T3 and free T4: the actual usable hormone circulating, not just the storage form
  • Reverse T3: whether stress is diverting your T4 toward an inactive pathway
  • TPO and thyroglobulin antibodies: whether an autoimmune process is involved
  • Ferritin, selenium, and zinc: the nutrient cofactors that determine whether conversion happens efficiently

 

Each of those has a different fix, and most of them are findable with the right labs.
